Registered Nurse - Case Manager

Full Time, 2nd Shift, Monday-Friday 12:00pm-8:00pm

Provides and directs the provision of home nursing care, based on agency policies and procedures, through the competent application of the nursing process.
Education and Formal Training: Associate degree in nursing, RN licensed in the State of North Carolina. Current CPR or obtain within 30 days of hire.
Work Experience: Minimum of one year of nursing experience preferred.
Must have a valid Driver License, reliable transportation, and a satisfactory driving record. 
Knowledge, Skills, and Abilities Required: Demonstrates a high level of mental and emotional tolerance and even temperament when dealing with ill people; uses tact, sensitivity, sound judgment, and a professional attitude when relating with patients, families, and co-workers at all times. Actively functions as a patient advocate. Must be able to quickly develop thorough knowledge of home health regulations and all relevant policies.

Registered Nurse - Case Manager

PRN, 1st Shift

Physical Therapist

Full Time, 1st Shift, Monday-Friday 8:00am-5:00pm

Provides and directs the provision of home physical therapy, based on agency policies and procedures, through the competent application of the physical therapy process.
Education and Formal Training: Successful completion of a PT program approved by the APTA or the Council of Medical Education and Hospitals of American Medical Association or both. If licensed or registered prior to 1966 meets the alternative qualifications cited in the Medicare COP, Section 405.l2 or (1)(2)(3)(4). Licensed in the State of North Carolina as a Physical Therapist.  Current CPR or obtain within 30 days of hire.
Work Experience: Minimum of one year experience as a Physical Therapist preferred but new graduates will be considered.
Additional Requirements: Current NC Driver License, satisfactory driving record, and reliable transportation.

Physical Therapist Assistant

Full Time, 1st Shift, Monday-Friday 8:00am-5:00pm

Carries out treatment, implementation and documentation of client status for home health clients under the direction of the registered physical therapist.

Education and Formal Training:Graduate of a two-year college-level program approved by the American PT Association or has two years of appropriate experience as a PTA and has achieved a satisfactory grade on a proficiency exam conducted, approved or sponsored by the U.S. Public Health Service. Licensed as a physical therapy assistant in North Carolina.

Work Experience: Minimum of one year experience as a physical therapist assistant.

Knowledge, Skills, and Abilities Required: Demonstrates a high level of mental and emotional tolerance and even temperament when dealing with ill people; uses tact, sensitivity, sound judgment and a professional attitude when relating with patients, families, and co-workers at all times. Actively functions as a patient advocate. Must develop knowledge of current home care emergency policies and procedures, fire, safety, disaster, and infection control policies.

Occupational Therapist

Full Time, 1st Shift, Monday-Friday 8:00am-5:00pm

Provides and directs the provision of home occupational therapy, based on agency policies and procedures, through the competent application of the occupational therapy process.

Education and Formal Training:  Registered by the American Occupational Therapy Association or successfully completed the educational requirements of OT and licensed in the State of North Carolina.

Work Experience: Minimum of one year experience as an Occupational Therapist.

Knowledge, Skills, and Abilities Required:  Demonstrates a high level of mental and emotional tolerance and even temperament when dealing with ill people; uses tact, sensitivity, sound judgment and a professional attitude when relating with patients, families, and co-workers at all times.  Actively functions as a patient advocate.  Must develop knowledge of current home care emergency policies and procedures, fire, safety, disaster, and infection control policies.
